Modification to NRAI application form? Is it okay?

Friends,

I was in the process of applying for NRAI membership and had my form signed by two members in Feb 2016. However I misplaced the form while shifting offices and left it since then and got busy with other priorities. I happened to locate the form couple of days ago. I am keen to complete the process and would like to send the application to NRAI. However, some of the details need to be changed, like the date in the form, email id, and my occupation details (have changed jobs and hold a new position now). Is it okay to correct the details using a whitener? Or should I fill up a new form?
