Hello Mukuluttam,

The best way to get this Transfer done is to get in touch with the Secretary General of CPRA. They will have the format with them. They might charge a few bucks to provide you with the letter (not sure though). Get the NOC, submit to UPRA & get enrolled into UPRA.
