I Still not understand what is the meaning of The Form in which the licence is required please help me to understand.

there are many different types of arms licence etc. ie dealers, and so on.
if you want as an individual to own a firearm then you will need to say Form III
say pistol/rifle/shotgun etc as NPB .32 pistol/ NPB .32 ammo etc see the last link, part C No 13 for arms and ammo
